Surgical Management of Pancreatic Cancer

Abstract
The hospital of 95 patients admitted and undergoing operation at the surgical department of the Seoul National University Hospital from 1976 to 1982 were reviewed and the result were as follows; 1) Sixty seven percent of the pancreatic cancer was found in the head, 22% in the body and tail and \\% in the entire portion of the pancreas. 2) Patients were classified according to the criteria used by Friesen. Nine cases were stage I ; 10 cases, stage ]I; 17 cases, stage 1 ; and 59 cases, stage IV : It means that eighty percent of the patients were stage I and W. 3) Fifty three percent had a previous history of diabetes mellitus (glucose level, more than 140/200 mg%) or a impaired glucose tolerance test (more than 100/140 mg%) 4) Diagnostic accuracy of Ultrasonography and Computerized tomography were 68%, 90% respectively. 5 ) Of the 95 cases of pancreatic cancer resection was performed only in 17 cases(l8%), palliative procedure in 53 cases(56%), open and closure in 25 cases(26%). In the group of 17 patients undergoing operation intended for cure, twelve had pancreaticoduodenectomy, two total pancreatectomy and three distal pancreatectomy. Among 53 patients undergoing palliative procedure, biliary bypass was done in 30 cases, enteric bypass in 9 cases, biliary and enteric bypass in 14 cases. 6) Operative mortality and morbidity rates were developed in 2%, 13% of the cases respectively. 7 ) The average survival was more than 21 months for the group of patients having resection, 5.0 months for the patients performed palliative procedure, 5.7 months for open and closure group, and ]8.6 months for stage 1, 33.3 months for stage Jf, 11 months for stage I , and 3.2 months for stage IV. Above data showed increased survival in the resection group compared with previous study19) (16 months). Although new diagnostic method was used, resectability and survival rate were not increased significantly. So new diagnostic tool was needed for early detection of pancreas cancer.
